Stumbled into the book title "What Do You Care What Other People Think" but found only the second part of it. It came out to be Feinman's personal account of Challenger investigation.
2 years ago I read the Columbia investigation official report. Indeed they stated that they have discovered many organizational failiers that were pointed out by the Challenger commision back 20 years ago.
And it is funny that he found the software work being the most properly done. (But they did complain to him that management was trying to cut on tests for their tests never failed, so I do not know what happened since then).
